Vendor Registration Form
The District's Vendor Registration form is a substitute Internal Revenue Service W-9 form that enables collection of information required to comply with federal income tax laws. Please be sure to include on the form the commodity codes for the goods and/or services your company provides. To request a Vendor Registration form, please send your request via email to VendorRegistration@WaterMatters.org or by calling (352) 796-7211, ext. 4154.

Vendor Electronic Payment Authorization Form
